This questionnaire contains various questions regarding your feelings and beliefs about relationships. Please take some time to answer each of the questions carefully. There are no right or wrong responses to these questions, so please respond freely to the items.

This survey is only for people currently residing in the US. The survey is anonymous except for participants who wish to participate in a raffle for 2 $100 prizes who will have to disclose their e-mail addresses. The web survey will be hosted on a secure server and the IP addresses and all identifying information will not be collected in the same file as the responses. Because the researcher is interested in examining the patterns of responses over a large number of people, you will in no way be singled out and identified with the responses you provide. The responses will be kept in secure locations until potential identifying information such as their partners name is cleansed from the dataset.
There may be some risk associated with participating in this survey. More specifically, some participants may experience discomfort when answering survey questions if they consider the information to be sensitive or stressful. If you do experience distress or discomfort as a result of participating in this survey, I encourage you to contact your local mental health professional or one of the following organizations:

Your participation in this study is voluntary and you may refuse to participate. You may withdraw at any time during this study without penalty or click on the skip button and omit any items that you do not want to answer again with no penalty. The survey takes about 30 minutes to complete. If you so choose at the end of the study you can enter a raffle drawing awarding $100 each to two randomly selected participants.
